We participate with most major insurance carriers including Anthem Blue Cross/Blue Shield, Cigna, Aetna/USHC and most HMOs and PPOs. We also accept Medicare and Medicaid. If you are not insured or underinsured, call our business office at 804-285-1884. We will work together with you to establish a payment schedule that is mutually acceptable.

If it is your first appointment or your insurance information has changed, you will need to fill out an insurance information form and we will make a copy of your insurance card if you have one. We submit forms to your insurance company and co pays are due at your appointment. You are ultimately responsible for payment.
